Making a Referral to Occupational Therapy:
If you are a Teacher, and Administrator, or a Parent... The process is the same. There is a procedure that should be implemented BEFORE you contact the Occupational Therapist to refer a student with a signed Permission form.
Step 1: Take data for 2 weeks
Below is a button that holds a packet full of intervention ideas. These interventions are Tier 1 (universal) and Tier 2 (targeted) interventions (as part of the Utah State Board of Education's Multi-Tiered System of Supports initiative.)

The school teams need to implement various interventions and strategies from this packet- in an effort to have the data to guide the use of intensive services and strategies we use (like Occupational Therapy), in conjunction with FAPE (Free and Appropriate Public Education).
However, an intervention period should not unnecessarily delay a full OT evaluation if the student does not respond to the intervention. When in doubt, please consult with the OT assigned to your school.
Step 2: Consult with the OT
Contact the Occupational Therapist to receive assistance, recommendations, and adjustment as the strategies and interventions are used. Intervention period should not unnecessarily delay a full OT evaluation if the student does not respond to the intervention. When in doubt, please consult with the OT assigned to your school.
Step 3: Fill out the Referral Form & Permission to evaluate
The form is on page 39 of the packet. Send a copy of the data and the forms to the OT department so that they know what types of evaluations to use.
